Pharmaceutical Care Awards 2015 - Entries by 30 April - Ceremony 18 June

The Pharmaceutical Care Awards are a unique awards programme that recognise innovative practice and service developments that have been shown to improve the health of patients through better use of medicines. Past winners have come from all pharmacy sectors, entries can be from individual pharmacists or those working in multidisciplinary teams.

The finalists

The six finalists for the Pharmaceutical Care Awards 2015 have just been announced, the projects chosen to be presented are:

  • Improving and Maintaining Medicines Reconciliation on Admission at North Bristol NHS Trust
  • Minimising Polypharmacy and Adverse Drug Reactions in the over 75 years
  • Blood Borne Virus (BBV) Testing in Community Pharmacies – Integrated care model
  • Care Home Support Team (CHST) Project
  • Nursing Home Outreach Project
  • Operation Pharmacist: Ending the Highs and Lows of Diabetes

Attending provides a great opportunity to learn from leading examples of pharmaceutical care, be inspired by innovative cross-sector projects and support some of the teams who are driving the profession forward.

Places are limited, so join us at The Royal Society, London on Thursday 18 June to see these great projects presented.

Best practice symposium and presentations

The six finalists who have been shortlisted by the judges will have their presentations converted into poster presentations and displayed for your viewing pleasure. The finalists will also take part in a Q&A session with The Pharmaceutical Journal’s Executive Editor, where they will answer questions about their projects and showcase to key members of the profession the difference that pharmacists and pharmacy teams make in improving patient care.

An expert panel of judges will select the winner of the 2015 Pharmaceutical Care Award, and the audience will have the chance to vote for the winner of the Audience Choice Award.
