Introduction

One of the recommendations outlined in the NICE/NPSA technical patient safety solution on medicines reconciliation is that pharmacists should be involved in the medicines reconciliation process as soon as possible, as a step towards improving documentation of patients medicines on admission. It’s important that as part of our contribution to preventing medication errors and ensuring the appropriateness of patient’s current and future therapy, that we are able to obtain an accurate medication history, as this provides a foundation for medicines reconciliation process.
