The New Medicine Service and targeted MURs proposed for the community pharmacy contract will need to make sure that pharmacists working in secondary care are aware of the new services so that patients can be referred to community pharmacists appropriately. Specifically respiratory patients, patients on high risk medicines and those discharged from hospital are potential candidates for the new services. The WYLPF has a good working relationship with the LPC and wishes to make sure as many RPS and UKCPA members working locally in secondary care in the WYLPF area are fully aware of developments in primary care and are sharing best practice in the role out of the new community pharmacy contract.

Aims and objectives

The aim of the webinar is to highlight the essential cross sector communications required between primary and secondary care to support the new medicine service and targeted MURs.

The objectives of the webinar are:-

• To explore how hospital and community pharmacists can work together to support the implementation of the new medicine service

• To highlight appropriate referral of patients to community pharmacies in order to facilitate targeted MURs for discharge, high risk medicines, respiratory patients and the new medicines service which supports reduced hospital re-admissions.

• To share examples of good practice in implementing the new services
