We have developed two new tools to help you and your pharmacy team record and review near miss errors to prevent similar mistakes in future. Accompanying guidance is also available to help you use these new tools.
The first tool is the Near Miss Error Log which can be used to record near miss errors and information about the environment in which it occurred.
We have also developed a Near Miss Error Improvement Tool which enables you and your team to analyse and review information captured in the log. This allows trends and contributing factors to be identified so that an action plan can be developed with the aim of preventing similar near miss errors in future.
These new tools help support clinical governance in pharmacy and promote an open culture of reporting near miss errors.
