  • Medicines legislation describes the requirements which need to be on a legally valid prescription. Language is not specified
  • There is currently no law or act that specifies that prescriptions in Wales have to be bilingual.
  • If you’re not a Welsh speaker and can’t understand the prescription, our advice is to put patient safety first. It is your responsibility to find the best way to help the patient. If you are presented with a prescription you do not fully understand, this might be through translation services or informal networks
  • If Welsh is the language of choice for your patient, their needs must be respected and every effort made to support the patient – every patient should be treated with courtesy and respect to avoid potential discrimination.

Introduction

The Welsh Language Act 1993 gives the Welsh and English language equal status in public life in Wales. Bilingualism is an integral feature of Welsh life. A Welsh language or bilingual service is vital for the welfare of Welsh speaking patients, according to an enquiry by the Welsh Language Commissioner.

The Royal Pharmaceutical Society (RPS) in Wales recognises the importance of access to Welsh speaking pharmacists for people living in Wales and appreciates the value of effective communication to help people get the most from their medicines.

We believe that pharmacists need to serve the language needs of their local communities, which may differ from community to community.

We are supportive of pharmacists conducting a consultation in Welsh, if both patient and pharmacist have sufficient fluency in the language and this is what both parties want. This consultation may be a natural bilingual conversation and may revert to English for technical issues and to confer understanding. 

If you are a Welsh speaking pharmacist and would like support in developing consultation skills through the medium of Welsh, WCPPE have developed a Consultation Skills Distance Learning Pack in Welsh. The pack aims to support pharmacists in conducting effective consultations which integrate a patient centred approach to everyday practice.

Cyflwyniad

Mae  Deddf  yr  Iaith  Gymraeg  1993  yn  rhoi  sail  gyfartal i’rGymraeg a’r Saesneg mewn bywyd cyhoeddus yng Nghymru. Mae dwyieithrwydd yn rhan annatod o fywyd yng Nghymru. Mae gwasanaeth iechyd Cymraeg neu dwyeithog yn hanfodol ar gyfer lles cleifion Cymraeg eu hiaith, yn ol ymchwidiad gan y Comisiynydd y Gymraeg. Mae’r Gymdeithas Fferyllol Frenhinol yng  Nghymru  yn  cydnabod pwysigrwydd  cael  mynediad   at fferyllwyr sy’n siarad Cymraeg ar gyfer pobl sy’n byw yng Nghymru, ac yn gwerthfawrogi gwerth cyfathrebu effeithiol i helpu pobl i gael y gorau o’u meddyginiaethau. Os ydych chi’n fferyllydd sy’n siarad Cymraeg ag am gymorth i ddatblygu eich sgiliau ymgynghori trwy gyfrwng y Gymraeg mae’r WCPPE wedi datblygu pecyn dysgu o bell  Sgiliau Ymgynghori yn y Gymraeg.

Credwn fod angen i fferyllwyr wasanaethu anghenion ieithyddol eu cymunedau lleol, a allai wahaniaethu o un gymuned i’r llall.

Rydym yn cefnogi fferyllwyr sy’n cynnal ymgynghoriadau yn Gymraeg, os yw’r claf a’r fferyllydd yn ddigon rhugl yn yr iaith, ac mai dyna yw dymuniad y naill a’r llall. Gallai’r ymgynghoriad hon fod yn sgwrs ddwyieithog naturiol ac fe allent droi i’r Saesneg argyfer materion technegol ac i sicrhau dealltwriaeth.

Treating Welsh speakers with dignity and respect

The inquiry by the Welsh Language Commissioner into the use of the Welsh language in primary care (My Language, My Health) highlighted the need for pharmacists and other health professionals to treat Welsh speakers with dignity and respect.

It is important that patients are treated with courtesy at all times and that pharmacists adhere to the standards of conduct, ethics and performance set out by the GPhC. If Welsh is the language of choice for your patient, this must be respected and every effort made to support the patient, in order to make them feel fully included in decisions about their health care and to ensure theyare protected from potential discrimination.

Fe wnaeth yr ymholiad gan Gomisiynydd y Gymraeg i ddefnydd  y Gymraeg o fewn gofal sylfaenol (Fy Iaith, Fy Iechyd) dynnu sylw at yr angen i fferyllwyr a gweithwyr iechyd proffesiynol eraill i drin siaradwyr Cymraeg ag urddas a pharch. Mae’n bwysig bod cleifion yn cael eu trin â chwrteisi bob amser a bod fferyllwyr yn cadw at safonau ymddygiad, moeseg a pherfformiad a amlinellir gan y Cyngor Fferyllol Cyffredinol. Os Cymraeg yw dewis iaith eich claf, rhaid parchu eu hanghenion a gwneud pob ymdrech i gefnogi’r claf, er mwyn gwneud iddynt deimlo’n rhan lawn o benderfyniadau am eu gofal iechyd ac i sicrhau eu bod yn cael eu hamddiffyn rhaggwahaniaeth posibl.

Dispensing prescriptions

Medicines legislation describes the requirements which need to be on a legally valid prescription. Language is not specified.

The opinion of the Welsh Pharmacy Board is to put patient safety first. It is your responsibility to find the best way to help the patient. If you are presented with a prescription you do  not fully understand, this might be through translation services or informal networks. Some local health boards choose LanguageLine.

A Welsh toolkit for community pharmacists which addresses key Welsh phrases and covers the technical issues of dosage and cautionary labels, accredited and endorsed by the Welsh Language Commissioner, could be a way forward. This is something we raised in our letter to the Welsh Language Commissioner.

Prescribing

Prescribers should consider patient safety when prescribing. Issuing prescriptions partly or solely in a language other than English may introduce time delays in being dispensed, so if, as a prescriber, you think that a time delay could put your patient at risk, we recommend that you ensure that your prescription is fully bilingual or in English.

Labelling

In the interests of patient safety, under Requirement 266(7) for packaging and package leaflets relating to medicinal products  in The Human Medicines Regulations 2012 it is stated that ‘Information given in English in accordance with this regulation may be given in several languages in addition to English, provided that the same particulars appear in all the languages used.’ In addition to ensuring that all pharmacists comply with these crucial legal requirements, medicines should be labelled in English or bi-lingualy including Welsh and English to ensure that if a patient is seen by a non-Welsh speaker these importantinstructions are understood.

Use of BNF Cautionary and advisory labels

RPS Wales are delighted that the March-September 2016 edition of the BNF includes comprehensive Welsh translations for each cautionary and advisory label. These labels have been produced by a team of specialists from Bangor University and Betsi Cadwaladr University Health Board, the team consisted of language specialists and pharmacists.

The work has been approved and endorsed by the All Wales Medicines Strategy Group (AWMSG), which develops resources to support prescribers and maximise health gain through the safe use and cost-effective use of medicines.

These labels are now available for pharmacists in order to increasing the amount of health support provided to individuals in Welsh within the NHS in Wales. RPS Wales and the Welsh Pharmacy Board is cognisant that many health care providers that a patient may interact with will not understand written Welsh, in order to ensure patient safety, prevent time delays  in treatment as well as conforming to the requirements set our in the Human Medicines Regulations 2012 it is vital that these labels are produced bi-lingual rather than Welsh only.

Presgripsiynu

Dylai presgripsiynwyr ystyried diogelwch cleifion wrth bresgripsiynu. Gallai ysgrifennu presgripsiwn yn rhannol neu’n gyfan gwbl mewn iaith ac eithrio Saesneg arwain at oedi wrth roi cyffuriau, felly os ydych chi, fel presgripsiynydd, yn credu y gallai oedi roi eich claf wrth risg, rydym yn argymell eich bod yn sicrhau bod eich presgripsiwn yn gwbl ddwyieithog neu yn Saesneg.

Labelu

Er budd diogelwch cleifion, O dan Ofyniad 266(7) am becynnu  a thaflennu o fewn pecynnau sy’n ymwneud â chynhyrchion meddyginiaethol yn Rheoliadau Meddyginiaethau Dynol 2012 dywedir Gwybodaeth a rhoddir yn Saesneg, yn unol â’r rheolaidd hwn, y gellir ei rhoi mewn sawl iaith wahanol yn ogystal â’r Saesneg, ar yr amod bod yr un manylion yn ymddangos yn yr  holl ieithoedd a ddefnyddir . ‘ Yn ogystal â sicrhau bod yr holl fferyllwyr yn cydymffurfio â’r gofynion cyfreithiol hanfodol hyn,...... er budd diogelwch..... dylai meddyginiaethau gael eu labelu yn Saesneg neu yn ddwyieithog yn cynnwys Cymraeg a Saesneg I sicrhau, pe bai claf yn cael ei weld gan rywun di-Gymraeg, y bydd y cyfarwyddiadau pwysig hyn yn cael eu deall.

Defnydd o labelai rhybuddiol a chynghorol y BNF

Mae’r Gymdeithas Fferyllol Frenhinol yng Nghymru yn falch iawn bod rhifyn Mawrth - Medi 2016 o’r Llyfr Fformiwlâu Cenedlaethol Prydain (BNF) yn cynnwys cyfieithiadau Cymraeg cynhwysfawr  ar gyfer pob label rybuddiol a chynghorol. Mae’r labeli wedi cael eu cynhyrchu gan dîm o arbenigwyr o Brifysgol Bangor a Bwrdd Iechyd Prifysgol Betsi Cadwaladr, roedd y tîm yn cynnwys arbenigwyr iaith a fferyllwyr.

Mae’r gwaith wedi cael ei gymeradwyo a’i gadarnhau gan y Grŵp Strategaeth Feddyginiaethau Cymru (AWMSG), sy’n datblygu adnoddau i gefnogi rhagnodwyr a gwneud y mwyaf o adnoddau iechyd trwy’r defnydd diogel a chost - effeithiol o feddyginiaethau.
