About this webinar
More than 23 million people in England now have an SCR, containing the medications, allergies and adverse reactions (and additional information that the patient may have requested) from the GP record, and rapid progress is being made with record creation. The SCR is starting to be used by Pharmacists across the country to support patients who require urgent or emergency care. This is a subject of key relevance to Pharmacy practice in Secondary Care to support medicines reconciliation. Find out what is happening in your area and other parts of the country.
Aims and objectives
The aim of this webinar is to inform Pharmacists about the availability of the SCR, what it is, and how it can be of benefit to pharmacists and their patients by enabling safer, more appropriate and more efficient care. This will be illustrated with case studies and audit data from the Pharmacy Service in Northumbria Healthcare NHS Foundation Trust, and also real world examples from across England.
