The
objectives of the sub-group are:
To
support the seamless pharmaceutical care principles:
- We
will support mobilisation of the workforce to embed greater
collaboration across all sectors including social care to improve
safety and patient experience;
- We
will shift the focus of hospital teams to respond to changes in the
delivery of care and transform access to medicines; and
- We
will ensure the development of the roles of specialised pharmacists
and pharmacy technicians leading on centralised all Wales Medicine
Services.
To
deliver the 2022 goals:
- Pharmacy
Teams embedded at cluster level;
- New
community pharmacy contract increases focus on medicines
optimization, health & wellbeing; and
- Transform
access to medicines and pharmaceutical advice to improve quality,
sustainability and value.
To
deliver the 2030 goals:
- Pharmacy
will manage all medicines as the experts in therapeutics, medicines
usage and optimisation.
- Community
Pharmacy contracts will compliment other professional group
contracts for the benefit of their cluster population.
The
Seamless Pharmaceutical Care sub-group meets on a bi-monthly basis
and aims to support the principles and deliver the goals linked to
Seamless Pharmaceutical Care theme of Pharmacy: Delivering a
Healthier Wales.
The
sub-group is jointly led by Lloyd Hambridge (Deputy Chief
Pharmaceutical Officer in Welsh Government and Primary Care Cluster
Lead in ABUHB) and Jonathan Simms (Clinical Director of Pharmacy in
ABUHB), with wider membership composed of representatives from
community and domiciliary care, NHS 111, community pharmacy, health
boards, clusters and hospital pharmacy.
