One of the key skills of a pharmacist is to perform a clinical assessment or clinical check before medicines are prescribed, supplied or administered. Clinical checks involve identifying potential pharmacotherapeutic problems by gathering and evaluating all relevant information, including patient characteristics, disease states, medication regimen and, where possible, laboratory results.
